This form is for individual players to submit an expression of interest in joining a team in our Monday Night Social Mixer Competition.

When completing this form you will need to indicate a team preference, or both:

* Female only team - minimum age is 13yrs (born 2013)
* Mixed Team (female and male) - minimum age is 15yrs (born 2011)

Your details will be made available to any teams that are looking for players, and they will contact you direct.
